Description
Oaklands is a large detached house that is situated within walking distance of the city centre. We provide assessment and rehabilitation for up to eight adults with mental health conditions. We also support in the community for those moving on, under the Direct Payment Scheme. Individuals are supported to develop daily living skills and move towards more independent living. At Oaklands, all of the support planning is person centred which allows each individual to identify personal goals and needs and to decide how best to meet these. Social workers, families and other members of the multidisciplinary teams are very involved in each individual's support and development.
